Contiunous Hemofiltration in Children with Renal Failure and Refractory Edema

Abstract

|
Continuous atrteriovenous hemofiltration (CAVH) which was described by Kramer in 1977 provides an alternative method for the treatment of acute renal failure. Seven children with acute renal failure and refractory edema were treated with
continuous
hemofiltration. Five of the seven patients had acute renal failure with rrefractory edema and the remainder had acute renal failure with multiple organ failure. The duration of treatment ranged from 12 to 126 hours, with a mean of 57¡¾65 hours.
The
mean
ultrafiltration rate was 7.7¡¾4.4 ml/min and the mean weight loss during treatment was 4.7¡¾2.8Kg. Changes in blood chemistry before and after treatment were not significantly different except for BUN (92¡¾16 vs 66¡¾18mg/dl, p<0.05). We observed
initial
clinical improvement in all cases but two patients died from pneumonia and sepsis, developed later. There was one episode of blood line clotting and one case of hypotension during hemofiltration.
We concluded that continuous hemofiltration in children is a safe and effective method in the treatment of acute renal failure with intractable edema and multiple organ failure.
